Your privacy is important to GEM. All the personal information you provide is treated with the utmost respect and is not shared with 3rd parties.
Medical / Consent Form
When you arrive for your first appointment at GEM, you will be asked to complete a Medical / Consent Form. This form asks for basic contact information, details of your medical history and asks you to consent to treatment. It is a requirement of both the federation to which I am accredited and my insurer that I collect this information. It is also good clinical practice as the medical information you provide informs me if you have any conditions that would prevent treatment or mean that the treatment should be adapted.
In accordance with data protection legislation the contact/ medical information you provide me is securely stored for a minimum of eight years from your last treatment. You can ask to access this information at any point. We will not share this information with any third parties except in these circumstances:
- with named third parties with your explicit written consent
- with the relevant authority such as the police or a court, if necessary for compliance with a legal obligation to which we are subject e.g. a court order
- with your doctor or the police if necessary to protect yours or another person's life
- with the police or a local authority for safeguarding children or vulnerable adults
- with my federation, the FHT, or my insurance company in the event of a complaint or insurance claim being brought against me
- my solicitor in the event of any investigation or legal proceedings being brought against me.
You have the right to see and access the data we hold on you at GEM. If you are concerned that your data is being misused please contact me at GEM on gerald@ge-m.co.uk. If you do not find the response satisfactory please contact the Information Commissioner’s Office at https://ico.org.uk
