I give my consent to allow Gerald Elphicke to perform manual therapy on my body and to take me through relevant corrective exercises.

I understand GEM’s cancellation and rescheduling policy: there is no charge if you cancel more than 24 hours before your appointment. After this only half the cost of your appointment will be refunded and if you miss your appointment altogether you will be charged the full fee.
